Who handles this information?
This policy explains how enquiry information is handled by this site, in an Australian Privacy Principles-style format. The site is an information and enquiry service, not a clinic. Health enquiries can reveal sensitive information even when they contain no health history, so we treat the context of every enquiry with care.
This policy should be read with the collection notice immediately above the form’s submit button and the visible partner disclosure.
What information do we collect?
The form asks for first name, Australian mobile number, email address, state and postcode, age confirmation, an optional broad topic, an optional preferred contact time and sharing consent. It does not request a date of birth, a health history, symptom descriptions, photographs or a Medicare number. Do not send clinical documents through this site.
When an enquiry is submitted, the record also includes its creation time, a consent timestamp and the consent version share-v1. Referral metadata identifies the source site, the health category and an interim destination tag. The landing page is a path only, not a browsing history.
Why do we collect it, and do I have to enquire?
We collect these details to record your request and enable our referral partner clinic’s team to contact you about an assessment. The form requires age confirmation and actively selected sharing consent; its consent box is unticked when you first open it.
You may browse the pages without completing the form. If you do not agree to sharing, do not submit an enquiry — you can contact your own GP instead. We do not sell your information and do not use the form to request marketing consent.
Who may receive my details?
We currently pass enquiries to one referral partner clinic. The clinic’s identity and our commercial relationship with it are confirmed to you in writing before you book.
With your consent, enquiry details are shared with our referral partner clinic so its team can contact you about an assessment. Only if you later choose may information be shared with a service connected to that clinic. Sending an enquiry does not provide consent for any later service or clinical activity. The clinic explains its own privacy arrangements separately.
Where is information stored, and could it go overseas?
Enquiry records are stored privately in Lovable Cloud and are not available for public browsing. Ask us about hosting locations and any overseas processing before sharing details if that matters to your choice; we do not claim Australian-only storage without verification.
Private storage does not mean no authorised service provider can process information. Access procedures and safeguards apply to the people and services handling enquiries.
How do we protect sensitive enquiry information?
The form asks for minimal details, and direct public access to enquiry records is not permitted. Validation checks required contact fields and consent before storage. Clinical records created by a clinic are governed by that clinic’s separate arrangements.
No analytics or advertising trackers are used on this site. Consider whether other people can access your device, email or mobile notifications, and tell the clinic your contact preferences before discussing sensitive matters.
How long do we keep enquiries?
Enquiries are kept only as long as needed for the referral purpose and any applicable recordkeeping obligations, then deleted or de-identified. You can request deletion of your enquiry details and withdraw consent for future sharing at any time; a request may require proportionate identity checks, and legal obligations may affect what can be removed.
How can I access or correct my information?
You may request access to your enquiry information or ask for inaccurate details to be corrected. Provide only enough information to identify the relevant enquiry; do not send extra health information to establish identity.
If details have already been shared, ask whether a correction or deletion request can also be passed to the recipient. A clinic may have its own recordkeeping responsibilities, so a request to this site does not automatically change clinical records held elsewhere.
How do I raise a privacy complaint?
First raise the concern with us through the contact channel in this policy. Explain which part of the enquiry handling concerns you and what response you are seeking, without adding unnecessary health details.
The Office of the Australian Information Commissioner provides guidance on privacy rights and making a complaint, and may be able to assist if our response does not resolve the concern.
